Patient Resource Library

HPA/LiveWell has created a patient resource library to provide patients with the resources that clinicians have recommended, all in an easily accessible, secure place that will be managed by our library vendor. The patient resource library does not contain any sensitive patient information or Personal Health Information (PHI).

You will require a unique username and password to access the patient resource library. In order to create the unique username and password, you will need to provide the library vendor with your email and your first and last name.

By checking the box to agree to the terms and conditions, you acknowledge and agree to the following:

• The secure patient resource library requires a unique username and password to access.
• I agree to share my email address and first name with HPA/LiveWell and the library vendor so that the library vendor can manage my account.
• I authorize HPA/LiveWell and the library vendor to communicate with me using my email address if necessary, pertaining to my access to the library.
• My email address, first name and last name will be used by the library vendor only for the purpose of creating my username and password, and for no other purpose.

I have read and understand the Terms and Conditions for Patient Resource Access. All my questions have been answered to my satisfaction. If I have additional questions, I will direct them to the HPA/LiveWell Administrative Team.

I hereby agree to allow HPA/LiveWell and its library vender to view my email address, first name and last to manage my access the patient resource library.
